- The radiation protection officers have been granted the necessary powers to fulfil their duties.
- The radiation protection officers have the necessary expertise in radiation protection.
- If no radiation protection officer has been appointed, the person submitting the application has the necessary expertise in radiation protection.
- The personnel required for the safe performance of the activity are available.
- It is ensured that the persons working have the necessary knowledge and skills with regard to the possible radiation hazard and the protective measures to be applied.
- The equipment is available and the safety regulations are complied with.
- This is a justified type of activity and there are no other public law regulations to the contrary.

Required documents
- Test report of the expert
- Radiation protection instructions in accordance with § 45 of the Radiation Protection Ordinance (StrlSchV)
- Plans, drawings of the structural and technical radiation protection facilities (e.g. floor plan of the X-ray room, site plan)
- Extract from the commercial or partnership register
- Copy of the notification of the person responsible for radiation protection
- Copy of the letter on the transfer of duties and responsibilities to the radiation protection officer by the authorised representative
- Current certificate of good conduct of the person responsible for radiation protection or the authorised representative and the appointed radiation protection officer
- Copy of the letter of appointment of the radiation protection officers
- Copy of the certificates of competence of the appointed radiation protection officers (or of the person responsible for radiation protection if no radiation protection officers have been appointed) including proof of the last update
